Abstract:

Interprofessional complex care targets the individual and community levels of social support in order to improve the health and well-being for at-risk individuals. Our interprofessional team includes students from nursing, social work, and health studies majors who work alongside clients who have been underserved by the healthcare system, experience multiple hospitalizations and have limited social support. We aim to help our clients reduce barriers to achieving optimal health.
